Emergencies are transported to Noble Hospital by Public Safety or ambulance.

Please have your Physcian fill the forms out and return them to Westfield State College Health Services, 577 Western Ave., Westfield, MA 01086-1630. If you have any questions please contact the Health Services Office. Note that all full time undergraduate residential students are required to have meningitis vaccine or submit a signed meningitis waiver form.

Hours of Operation: 8:30am - 5pm Monday through Friday

Weekends the Department will be closed. Appointments are not necessary.

Eligibility: All full time undergraduate students, regardless of health insurance, can utilize the Department's services. There are no fees charged for visits and medications provided. The Department provides minimal First Aid care and certain preventative vaccine clinics as a service for employees, faculty and staff. Requirements: All full time undergraduates are required to submit a Health Form which includes a physical exam and immunizations.

Confidentiality Statement: Medical records are strictly confidential. Information will not be released without written authorization from the student or a court order. Authorization forms are available at Health Services. See the College's notice of Privacy Practices.

College Health Insurance: Students who purchase this insurance can obtain claim forms bygoing to http://www.universityhealthplans.com and clicking on claim forms. This insurance provides accident and sickness coverage from September 1 through August 31 of the following year. Prescription privileges allow for a $10.00 co-pay for generic and $15.00 co-pay for brand name presriptions at certain pharmacies. The College Insurance under Nationwide Life Insurance Company offers a preferred provider directory on their website at www.consolidatedhealthplan.com or www.phcs.com. A list of providers can also be requested by calling PHCS at 1-866-559-7427 or Consolidated Health Plan at 413-733-4540 or 1-800-633-7864.

Absence Notices: The department will no longer notify professors of absences due to federal regulations on the privacy of protected health information.
